Hi, I’m Dr. Jaclyn Rabito here, from Let’s Move Physical Therapy, and today we’re talking about what to expect on your initial physical therapy evaluation. This is for cash pay or insurance. Your evaluation will always take place in a clinical setting. I’ll be asking you a bunch of questions. We’ll do some outcome measures as well, and that’s the conversation that’ll take place during your evaluation.
So when you walk in you’ll come up the ramp, and through the door. I’ll have your papers or your initial paper information here or over here for you for your evaluation. I’ll have you fill that out. I’ll always need a copy of some form of I.D. of your choosing as well when you come, and once these forms are filled out, I’ll ask you a million and one questions about why you’re here for an evaluation.
And again, we’ll do the outcome measures for the specific joint or reason for your evaluation, and this usually takes anywhere from 30 to 60 minutes. It’ll be a lot of talking, a lot of me doing range of motion manual muscle testing, outcome measures, functional measures. So it’s a lot, a lot of data collection, so just be ready to hit everything that day.
Also, if you have a lot of pain in the joint that we’re looking at, you have to let me know during the evaluation. There are certain tests that always stop when you have pain. And if it gets to the point that you cannot continue because you’re in so much pain, please tell me that because that is a part of the evaluation that you can’t necessarily finish certain tasks or functional activities that I would like to test as well.

At your initial evaluation, if you have a copay, you’re expected to pay your co-pay or if your cash payer, you’re expected to pay the rate for a cash evaluation at the time of service and when you take cash check or Zelle at this time. I will always give you a receipt and you’re always welcome to ask for a copy of your initial evaluation or any of your notes at any time.
